Can not Add Record in the DDL when using the Meeting Minutes Data Definition .

Description

1. Add Dynamic Data Lists portlet to a page.
2. Click "Add a List" in the bottom of the portlet.
3. Fill in the new list and select the 'Meeting Minutes'.
4. Add Record.

It will show a blank instead of the details about Meeting Minutes.At same time the console throws errors.

(Work as 6.1.10 EE GA1.)

Environment

Tomcat 7.0 + MySQL 5. Portal 6.1.20 EE GA2. Tomcat 7.0 + MySQL 5. Portal 6.1.x GIT ID: 1a8a706674247fc72355c506fabbe5bc0b2a9bff. Tomcat 7.0 + MySQL 5. Portal 6.2.x GIT ID: 4fbe1e8940bd1e26faeca3bf1541a7f777b061f4.

PASSED Manual Testing following the steps in the description.

Reproduced on:
Tomcat 7.0 + MySQL 5. Portal 6.1.x EE GIT ID: 1a8a706674247fc72355c506fabbe5bc0b2a9bff.

It will show a blank instead of the details about Meeting Minutes. At the same time the console throws errors.
