Expanding all pages in Manage Pages does not work

The "Expand All" option in Manage Pages does not work because it should have been removed. Using "Expand All" was very bad on performance, so each node node is now dynamically loaded when the node is expanded. When this change was made, the expand all option should have been removed also, but was not. So, this updates removes the "Expand All" text.
