When showing Categories in the Category selection widget, show Vocabulary name

When showing the selected Categories in the Category selection widget, both Vocabulary name and Category name should be shown (e.g. Region:ON).

It's otherwise impossible to tell from the UI widget which vocabulary was used. In fact multiple vocabularies may define the same categories.
