Go to configuration
User Clicks on [Form Fields]
User Clicks on [+] to add a new field
User selects [Radio Buttons] from the type dropdown menu
User types in a name for the [Name] field
User flags the [Optional] checkbox
User enters [1,2,3,4,5] in the Options text field
User Clicks on [+] to add a new field
User selects [Paragraph] as the type
User enters content in the text field
User Clicks on [+] to add a new field
User selects [Check Box] as the type
User types in a name in the [Name] field
User flags the [Optional] checkbox
User Clicks [Save]
User Clicks [Return to Full Page]
User clicks wrench icon
User clicks Configuration
User can no longer add more form fields
