Breadcrumb shows wrong when selecting document for Upload field

Reproduction steps:

• Go to Liferay site > Documents and media > Document Types
• Add a new document type with web content field and Upload field
• Add a document with the new dm type
• Click the Select button for the upload field
• Assert Breadcrumb on the Select File page

Actual Results: Breadcrumb on the Select File page shows as:

Sites and Libraries > Liferay DXP > com.liferay.dynamic.data.mapping.form.web > Forms > {Document name}

Expected Results: Breadcrumb on the Select File page shows correctly, like

Sites and Libraries > Liferay DXP

NOTE: Breadcrumb shows correctly when selecting web content.
