UI breaks on failed login when using multi-factor auth

    Description

      Steps to reproduce: 

      1. Login with Administrator
      2. Go to Control Panel » Instance Settings » Multi-Factor Authentication
      3. Check "Enabled" then update at the bottom. 
      4. Open a new private/incognito window
      5. Try the login with an incorrect password

      Expected Result: The UI continues to work

      Actual Result: The UI breaks
