Organizations do not save addresses and other contact information

Create an organization. Add address. Save the changes.

View all organizations, and the correct country and city show up in the view all list.

Edit the organization and add a phone number and save. Click to see the address, and the address is deleted/disappeared.

Attached image is a series of screenshots showing the problem. Looks like this happens at the 5th level down in an organizational structure..not sure i see it at the higher levels
