Steps to Reproduce:

• Open the URL
• Goto Control panel - Users and Organizations
• Using Jaws
• Navigate to the Organizations tab
• Plus sign the "+ " button and press enter key
• input the name
• Tab through the "Topic" edit field and observe

Expected Result:
The label "Test" should be associated with the edit field, SR should announce as "Test edit field"

Actual Result:
Topic edit field missing accessible label, SR is not announcing the label
Note: Same issue for "Tags" edit field
