Issue with Multiple Portlets on the Same page

I'm running into several cases where putting multiple portlets on the same page is causing them to crash. I am attaching a sample portlet to help recreate the issue.

To recreate:

1.) Deploy the attached includes-portlet to a 6.2 M6 environment
2.) Make a new page called "Test"
3.) Add the newly deployed "Include Test" to the new page twice and then refresh the page.

At this point both instances of the portlet crash. I've observed this behavior with several of our custom portlets.
