Form page navigation does not work for Form portlet

Steps to Reproduce:

1. Go to Site Admin > Content > Forms
2. Add Form with 2 pages and a field on each page
3. Add Form portlet to the Welcome page
4. Click on the form navigation tab to go to Page 2

Expected Result:
Page 2 of the form is loaded.

Actual Result:
Page 1 field still displayed.
