Forms - "unknown-error" when populating form field

Steps to Reproduced:

1. Go to Site Admin > Forms
2. Add Form with text field
3. Input "Name"
4. Save field
5. Save form
6. Add Form portlet to the Welcome page
7. Select created form
8. Populate text field
9. Submit

Expected Result:
Text field can be populated without error and Form can be submitted.

Actual Result:
Text field gives error message: "unknown-error" and Form cannot be submitted.

This also occurs when using Select, Radio, and Checkbox field types.

Tomcat 7.0.62 + MySQL 5.5.44. Portal master GIT ID: 56929bafe533370a44b5e7f4dc7532b0a4759b15.
