Date field predefined value does not function as expected

Steps to Reproduce:

1. Go to Liferay > Content > Forms
2. Add New Form
3. Input Form Name
4. Add Field
5. Select Date
6. Input Question
7. Click on "show-more-options"
8. Input predefined value
9. Save field
10. Save Form
11. Go to Liferay > Welcome page
12. Add Form portlet to the page
13. Select Form > Created Form
14. Save
15. Close configuration
16. View Date field value
17. Go to Liferay > Content > Forms
18. Edit Created Form
19. Edit Date field
20. Remove the Predefined value

Expected Result:
At step 7: There should only be 1 predefined value field
At step 16: When viewing the value in the Form portlet, the predefined value should display
At step 20: Form editor can successfully remove the predefined value

Actual Result:
At step 7: There is 2 predefined value fields
At step 16: The predefined value does not display
At step 20: Form editor cannot remove the predefined value
