Cannot delete DDL via management toolbar

Steps:

1. Create new DDL
2. Check the DDL
3. Delete it via management toolbar by trash icon or action menu

Expected result: DDL should be deleted

Actual result: DDL is not deleted, error in console

TypeError: form.queryString is not a function

Workaround: delete it via action menu (only one)
