Steps to Reproduce
Scenario 1

1. Start a clean bundle
2. Go to add a new user by cliking the Add button

Expect Result:
Add button rendered as expected

Actually Result
Add button shows as a blue icon without plus in it

Scenario 1

1. Add a new user
2. Go to edit the new added user

Expect Result
The edit button show with kebab

Actually Result:
Hard to see the edit button, icon shows up when hovering over it, without kebab

NOTE this issue not affect FF and Chrome
