Summary:

Forms is being lost when upgrading from 7.0 to Master (7.2).

Steps to reproduce:

1. Launch 7.0, go to Content > Forms;
2. Click to add a New Form;
3. Add some fields (Text Field, Paragraph, Select from list, Single selection, etc);
4. Add a public page containing this Form;
5. Submit some entries;
6. Upgrade the portal from 7.0 to Master (7.2);
7. Launch Master (7.2);
8. Go to the created Page and notice that Forms is not available;
9. Go to Content > Forms > open the Forms entries > notice that it is not filled as expected;
10. Try to open the Forms and observe the issue;

Expected result:

Forms should not be lost when upgrading from 7.0 to Master (7.2).

Actual result:

Forms is being lost when upgrading from 7.0 to Master (7.2).

NOTE: This issue is not reproducible when upgrading from 7.1 to Master (7.2).
